Enrollment Forms and Procedures
For each enrollment visit, a Family Worker Visit Form (NCR) form should be completed reflecting the topics discussed with the family, any information /materials given and any follow up needed. In addition to the forms listed below, the following forms are to be completed: Health and Nutrition History, TB Risk Questionnaire and the CACFP form (all found in Health/Nutrition section). A Watch Me Grow Permission to Exchange (NCR) should also be completed
The Returning Family Update (below) is completed at the enrollment visit with families returning for a second year. It insures that any changes in health, family or child data is noted and updated in COPA.
The Family Interest Survey is divided into 2 sections. The first section is to be completed either at the Open House, if a child is starting school at the beginning of the year, or at enrollment if a child is entering the program after the Open House has occurred. This section asks about family interest in family engagement opportunities offered by the program such as Home-School connections, policy council and parent committees. The second part of the form is to be completed at the second family visit and after 45 day screenings have been completed and a relationship exists to allow families to share more personal information.
