Classroom Emergency Board
Each classroom emergency board should be located next to a telephone whenever possible. IF the whole emergency board cannot be located next to the telephone then at minimum the yellow Emergency Telephone Procedures (see file below) should be posted next to the telephone.
The Following Information Must be on the Emergency Board:
1. Yellow Emergency Telephone Procedures
2. Poison Control Number
3. CPR Poster
4. First Aid Poster
5. Dental Emergency Procedures
6. Emergency Evacuation Route: mapped
7. A map and directions to the closest hospital

Compartment Clipboards
Each classroom has a compartment clipboard. This clipboard is to go with the class everywhere they go. The sign-in/sign-out for the classroom is to be kept on this clipboard. Stored inside of the clipboard are the Emergency Preparedness Quick Reference Sheets, Emergency Information Forms for each child and staff memeber, the blank Evacuation Relocation Posters, the red, green, and first aid color cards, whistle, pen, and sharpie.
Child Care Registration & Emergency Information Form
Each child has (2) copies of this form at the center. One is kept in the compartment clipboard as mentioned above and the other is kept in the child's file in the office. This form is update QUARTERLY with the parent/guardian and more often as needed. It should be redone annually. Anytime changes are made be sure that those changes are reflected on both copies.

First Aid Kit & Emergency Preparedness Bag Inventory
Any identified supply needs are reported monthly by center staff to their Center Director so they can be restocked. Each classroom has a first aid kit for the classroom and a portable first aid kit that goes outside in the red emergency preparedness bag. First Aid kits must be kept out of reach of the children at all times. Emergency preparedness bags are to be taken with the class every time you leave the building. The compartment clipboards (containing the emergency contact information Emergency Quick Reference Sheets) and any rescue medication must also be brought out in the emergency bag when leaving the classroom. Rescue medication must be kept out of reach of children at all times.
- Center Staff check the first aid kit and red emergency preparedness bag at minimum monthly and record this check on the Monthly First Aid Kit and Emergency Supplies Check List.
- Any identified supply needs are reported monthly by center staff to their Center Director so they can be restocked.
- The Center Director will report the needed supplies for their center(s) to the Health Coordinator and the Health Coordinator will provide the requested supplies.
- If a classroom is completely out of supply item in-between monthly checks they can request this supply when needed following the same process (ie. band-aids, ice packs, etc.). Or they can obtain it themselves from one of the supply storage locations by marking the taken supplies on the inventory list hanging inside the storage closet.
- The check list will be monitored for use by the Health Coordinator during regular monitoring visits to the centers. Center Directors will also monitor the use of the check lists during through their monthly Center Checklist.

Classroom Red Health Binders
Each classroom has a red binder that contains copies of the paperwork for all the children with health and nutrition conditions. This binder is to be organized alphabetically with a different section for each child with paperwork. It is to be stored in a confidential location. Only current paperwork is to be kept in the binder. If the paperwork changes or expires the old paperwork is to be removed. Originals of all the paperwork should be kept in the child's individual file in the office. The paperwork that should be in this binder includes, all health plans, medication paperwork, and special diet forms. For specific guidance on each of these forms, see the appropriate pages on this site.

Classroom Sanitation Procedures
These procedures must be followed by all staff. This provides a guidance on the minimum frequency of cleaning that should happen in all classrooms. Please see the 'Policies & Procedures' page under 'Health' for these procedures. Handwashing procedures and the Diapering Policy can also be found on the 'Policies & Procedures' page under 'Health'.

injury/incident reports
If any child receives an injury or any incident occurs which requires first aid treatment, medical treatment, or medical consultation, child care personnel will inform the parent/guardian of the injury/incident on the date of the injury/incident. An injury/incident form is completed. The form is a carbon copied form and copies can be found onsite at each center. The documentation on these forms must be objective and kept to the facts. Any injury/incident marked with an asterisk (*) must be reported to child care licensing.These forms are tallied by the Health Services Specialist and are analyzed for injury patterns.
Definitions:
Incident: A usually minor event or condition that is contingent on or related to something else or precipitates a crisis.
Example:
Injury: Damage or harm done to or suffered by a person.
Examples of Injuries that should be documented:
***Please note that if you have a child who is causing injury to other children you do not need to complete one of these forms for the child causing the injuries unless that child also gets hurt or is potentially exposed to bloodborne pathogens. For this child you should be working with the Social-Emotional support team and the family for a plan and intervention.
Definitions:

- A child has a spontaneous bloody nose that requires attention but was not caused by an injury.
Injury: Damage or harm done to or suffered by a person.
Examples of Injuries that should be documented:
- Child maltreatment
- Bites (complete form for the bitten, not the biter. If the biter breaks skin, potential exposure to blood borne pathogens paperwork should be completed)
- Falls, burns, broken limbs, tooth loss, other injury
- Motor vehicle injury
- Ingestion of non-food substances
- head injury
